An office of the public health department that provides immunizations, WIC services, and access to vital records including birth certificates, death certificates, and marriage/divorce documentation. Also provides tuberculosis screenings, STD testing, and international travel health consultations.

Description:

Program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding woman, woman who has just had a baby, or child less than five years of age who meet income requirements.

Provides treatment for bacterial STD's including chlamydia, gonorrhea, syphilis, pelvic inflammatory disease/infections as well as treatment for viral HPV.

Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Offers STI & HIV tests. Testing is also available for infections and other diseases that might affect the female reproductive system. Same day treatment for the diagnosis is offered, as well as PrEP treatments.

Eligibility:
Anyone 14 and older. Children are not permitted in the exam rooms or lobby.

M - F 8 am - 5 pm, Closed until 1 PM on the 1st and 3rd Wednesday of each month

Fees:
HIV testing is at no cost unless it is court ordered. Fees vary by test, but range from $40-$80. Cannot bill health insurance.
