Provides services that promote the achievement and maintenance of optimal health for all people in the community including death and birth certificates, WIC, pregnancy testing and birth control, prenatal home visitation programs, tuberculosis screenings and immunizations, early intevention, food handling permits, STD screening, and child passenger safety seat inspections.

WIC
Description:

The WIC program provides low-income women, infants and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or children less than five years of age. Must also meet income requirements.

Birth Control
Description:

Confidential education, counseling, and access to low-cost birth control options including oral contraceptives, IUDs, injections, patches, and condoms.

Tuberculosis Screening
Description:

TB skin tests for the diagnosis and prevention of tuberculosis.

Description:

Description:

Home visitation program with trained nurses that provide education and guidance regarding pregnancy and young children.

Early Intervention for Children With Disabilities/Delays
Description:

Provides an initial evaluation of disabilities and developmental concerns in children with subsequent intervention and support.

Families with children 0 - 3 with concerns for developmental delays or disabilities.

Description:

Sexually Transmitted Disease Screening
Description:

Testing and diagnosis for STD's including gonorrhea, chlamydia, hepatitis C, and HIV. Also provides treatment for bacterial STD's including chlamydia, gonorrhea, and syphilis.

Death Certificates
Description:

Provides copies of, and allows changes to, death certificates in central Utah.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.
Service Hours:
M - Th 7:30 am - 5:30 pm
Intake Procedure:
Requests can be made online, in-person and through the mail. Orders can also be made in person at the Richfield location.
Fees:
$30 for initial copy, other forms/copies vary between $15-30. See website for full cost structure.
