Description:
Helps children aged 17 and under gain access to specialized medical care by providing commercial airline tickets. Two parents/guardians may accompany the child on the flights.
Eligibility:
Child must be aged 17 and under. Child must have a confirmed medical appointment. Requested departure date must be at least 14 days in advance. Household income limit is 3 times the federal poverty guidelines provided by the US Department of Health and Human Services.
Service Hours:
M - Th 10 am - 4 pm
Intake Procedure:
Complete the flight request application, treatment site verification form, and flight request service dog application (if applicable) on the website.
