Semi-Independent Living Residences for Adults With Disabilities

Description:

Provides living facilities in a group home or host home for those who need more supervision and support development work and community living skills.

Eligibility:

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident, must have Medicaid Waiver.

M - F 8 am - 5 pm

Provides transportation services to help individuals access healthcare appointments, activities, and other social services. Can include bus passes or enhanced paratransit access.

Description:

Provides limited funds to individuals who serve as the primary caregiver for a qualifying family member with disabilities.

Eligibility:

Caretakers of individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident.

Developmental Disabilities Behavior Management Programs

Description:

Provides support in addressing behavioral problems for people with disabilities. Personalized consultation is given to families to help with serious or complex behavioral difficulties.

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident, must have Medicaid Waiver.

Description:

Provides support, supervision, socialization, personal care, training, and assistance in order to help people live as independently as possible.

Housekeeping Assistance

Description:

Provides services to help maintain a clean, sanitary, and safe living environment for persons with disabilities who are unable to complete chore activities on their own. Includes heavy household chores such as washing floors, windows, and walls, snow removal, lawn care, tacking down loose rugs and tiles, moving heavy items of furniture in order to provide a safe living environment.

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Description:

Services to allow families to make physical alterations and adaptations to the home as needed to help ensure the health and welfare of the individual.

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Adult In Home Respite Care

Description:

Promotes opportunities and provides support for those who care for people with disabilities, by providing a trained person to temporarily relieve parent(s) or care givers who provide care to the individual with disabilities.

Eligibility:

Individuals with qualifying disabilities in one of the three areas listed: intellectual disabilities or related development conditions (autism, down syndrome, genetic disorders, etc.), Acquired Brain Injury (ABI), or physical disabilities involving the functional loss of at least two limbs. Also, must be a resident of Utah and a U.S. citizen or legal permanent resident, must have Medicaid Waiver.
For this program, there are options if you are on the DSPD waiting list and do not yet have the waiver.
