Provides dental care, general medical care, mental health evaluations, counseling services, and a Diabetes management for those in the community who are uninsured, low income, or homeless. Accepts medical supplies donations.

Mental Health Evaluation
Description:

Provides mental health evaluation and treatment including assessments, counseling, and psychiatry services.

Eligibility:
Individuals who are uninsured or are below 250% of the federal poverty level.
Service Hours:
M - W 9 am - 4 pm; Th 9 am - 6:30 pm; F 9 am - 2 pm
Intake Procedure:
Call to schedule an appointment. Will accept walk ins from those in crisis and those recently discharged from Dixie Regional Medical Center.
Fees:
Counseling: $20; Medication Management: $10

General Counseling Services
Description:

Description:

Provides exams, x-rays, extractions, fillings, and root canals for those experiencing homelessness and low-income individuals who qualify. Must be at least 15 years old.

Eligibility:
Individuals who are uninsured or are below 250% of the federal poverty level. Must be at least 15.
Service Hours:
M - W 9 am - 4 pm; Th 9 am - 6:30 pm; F 9 am - 2 pm
Intake Procedure:
Call to schedule an appointment. Will accept walk ins on a first come, first served basis.
Fees:
Extractions: $40 - $50 per tooth; Fillings: $40 -$80 per tooth; Root Canals: $120 - $130 per tooth; Exams: $10

Description:

Will accept donations of new and gently used medical supplies.

Provides general and specialty medical care to children, adolescents, and adults who are uninsured, low income, or homeless.

Eligibility:
Individuals who are uninsured or underinsured and are below 250% of the federal poverty level.

Intake Procedure:

Patients are asked to donate of $10 if possible, but not required.

Provides diabetes medical care for individuals with diabetes or hypertension.

Eligibility:
Individuals who are uninsured or underinsured and are below 250% of the federal poverty level.
