Bear River Area Agency on Aging

Area agency in support of the aging senior citizen population. Services include home delivered meals, transportation, caregiver counseling, caregiver and care receiver support groups, general health insurance information/counseling, and a long term care ombudsman program.

Caregiver/Care Receiver Support Groups
Description:

The Caregiver Support Program primarily provides support group services to those caring for people who are 60 years or older. Care receiver support groups are also available for those in need.

Eligibility:
Care receivers must be over 60 years of age and in need of support. The caregiver must be actively involved in caring for the recipient.

Description:

Programs and support for the senior citizens of Box Elder, Cache, and Rich Counties to increase quality of life and extend independence. Services include in home services, state health insurance counseling, and information and referral.

Home Delivered Meals
Description:

Meals on Wheels is a program for senior citizens through local senior centers in Box Elder, Rich, and Cache Counties delivering hot, nutritious meals to eligible residents.

Adults ages 60 and older in Cache, Box Elder, and Rich counties.

M - F 8:30 am - 4 pm

Wellness Programs
Description:

Evidence based activities to include range of motion, strength, and a healthy lifestyle. In home services are also provided to assist seniors remaining in their homes.

Medicare Information/Counseling
Description:

Counselors offer Medicare beneficiaries objective information on benefits, coverage options, supplementing Medicare, prescription drug plans, and assistance programs, providing free, unbiased counseling via telephone, face-to-face interactions, public presentations, and media activities to help seniors make educated decisions. The Senior Medicare Patrol (SMP) empowers beneficiaries, families, and caregivers to prevent, detect, and report healthcare fraud, errors, and abuse through outreach, counseling, and education.

Age 65 and older or those with questions approaching Medicare age or their families or caregivers.

Caregiver Counseling
Description:

The Caregiver Support Program provides information, assistance, individual counseling, support groups, training, respite, and some medical equipment and/or supplies to individuals caring for family 60 years or older.

Care givers and/or the care receivers must be over 60 years of age and the care giver must be actively involved in caring for the recipient.

Long Term Care Ombudsman Programs
Description:

Advocates on behalf of residents of nursing homes, assisted living facilities, and other residential care facilities licensed by Utah Department of Health by voicing and resolving the concerns that elderly residents encounter. The Ombudsman helps residents to resolve complaints and concerns and informs residents and their families about the resident's rights.

Eligibility:
Any age resident of a nursing or assisted living facilities in Box Elder, Cache, or Rich Counties.

Senior Companion Program
Description:

Senior Companionship volunteers serve as companions to home-bound seniors by providing companionship, and errand assistance to grocery shopping, doctor appointments, pharmacy, etc. Companions receive a small stipend for their services.

Eligibility:
Volunteers in Senior Companions must be age 55 or older, have income below $2660 per month for an individual or $3606 per month for a two person household, and pass a physical and criminal background check. Senior Companions receive tax-free stipend.
