Home Nursing
Description:

Comprehensive home nursing care services (RN, CNA, ST, PT, OT, SW, PTA, ETC.), including administering medication, managing pain, wound care, injections, respite care (a break for family caregivers) outpatient physical, handling intravenous medications, and occupational therapy.

Hospice Care
Description:

Provides hospice care in home or at IHC facilities through Utah.

Eligibility:
Patient must have elected the Medicare Hospice Benefit. No age limitations. Patient must be certified by the Hospice Medical Director and attending physician, with a life expectancy of six months or less.
