Jordan School District Health Services

Increases awareness of the need for health insurance for all students and assists with applications for Medicaid and CHIP. Services include general health insurance benefits assistance, and navigator programs.

Navigator Programs
Description:

Helps clients understand, apply for, and enroll in health coverage through the online marketplace. Specifically helps students and their families get enrolled in Medicaid and CHIP.

Eligibility:
Students and their families in the Jordan School District.
Service Hours:
M - Th 8 am - 4 pm; F 8 am - 3 pm; Appointments available up to 1 hour before closing.
Intake Procedure:
Walk in or call to schedule an appointment.

General Benefits Assistance
Description:

Provides information and application assistance for social service programs like SSI, HEAT, DWS Eligibility, SNAP, and prescription discounts.

Eligibility:
Students and their families in the Jordan School District.
Service Hours:
M - Th 8 am - 4 pm; F 8 am - 3 pm; Appointments available up to 1 hour before closing
Intake Procedure:
Walk in or call to schedule an appointment with a Certified Application Counselor.
