A comprehensive medical clinic, providing general counseling, mental health evaluations, navigator programs, and general dentistry to individuals and families experiencing homelessness in the Salt Lake area.

Mental Health Evaluation
Description:

Has limited mental health evaluations. Staffed with APRNs and LCSWs on staff that can provide mental health evaluations under their scope of practice.

New patients walk in to establish care. Established patients can call for an appointment.

Family and Community Medicine
Description:

Provides primary medical care services to patients including acute care, specialty care for specific illnesses, and care management and coordination across the medical team.

Mobile Health Care
Description:

Can provide non-emergent mobile healthcare services to unhoused individuals.

Eligibility:
Unhoused people needing medical care that is not an emergency. Must have a barrier stopping them from making it in to the physical location.

Navigator Programs
Description:

Certified Application Counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Glasses/Contact Lenses
Description:

Can fill contact lens prescriptions and glasses lens prescriptions for the unhoused.

Eligibility:
Individuals experiencing homelessness or with an income less than 200% of Federal Poverty Level. Must have a referral from a doctor on staff.

2nd Tu of each month 5 - 5:30 pm

General Dentistry
Description:

Offers exams, x-rays, fillings, extractions, and dentures.

Eligibility:
Must be a registered medical patient of the clinic and be in good standing without a cessation of service order on file.

Description:

Counseling, crisis intervention, mental health, and substance use disorders for individuals experiencing homelessness. Also provides psychiatrists on staff that can help manage and prescribe medications.
