Description:
Provides end of life services to maximize comfort in patients with a life limiting illness. Hospice services include homemaking assistance, medication management, medical care supplies, therapy (physical, speech, occupational), counseling, and legal document assistance (wills, powers of attorney, and funeral planning). They also offer grief support resources to those in need.
Eligibility:
Adults over 18 with a life limiting illness. Must have an estimated life expectancy of 6 months or less.

M - F 8:30 am - 5 pm

Accepts Medicaid, Medicare and private insurance. Administration may decide on a case by case basis, the eligibility for Charitable Care.

Provides nursing care and services in the patient's place of residence. Can include their own home, their realtives home, retirement centers, etc. Services include pain management, wound care, and bathing assistance. Also provides therapy services and emotional support for the patient and their family.

Serves individuals over 18 who are disabled or elderly.
