Provides services that promote the achievement and maintenance of optimal health for all people in the community. Services include birth certificates, death certificates, and immunizations.

Immunizations
Description:

Provides immunizations for children, adolescents, and adults.

Eligibility:
Service Hours:
M - W, F 8 am – 4:30 pm; Th 9 am – 4:30 pm
Intake Procedure:
Walk in.
Fees:
Offers low or not cost immunizations for those who qualify.

Birth Certificates
Description:

Provides copies of and allows changes to birth certificates in Utah County.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.
Service Hours:
M – F 8 - 11:30 am & 1 - 4:30 pm
Intake Procedure:
Requests can be made online, in person, and through the mail.
Fees:
$25 for initial certificate, $10 for each additional copy; fee waivers available for those experiencing homelessness from local homeless service providers.

Death Certificates
Description:

Provides copies of and allows changes to death certificates in Utah County.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

M – F 8 - 11:30 am & 1 - 4:30 pm

Requests can be made online, in person, and through the mail.

$35 for initial certificate, $10 for each additional copy.

WIC
Description:

Program provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or child less than five years of age who meet income requirements.
Service Hours:
M, Tu, Th 8 am – 12:00 noon & 1 – 5 pm; W 10 am – 2 pm & 3 – 7 pm
Intake Procedure:
Call or apply online to schedule an appointment.
