Description:

Eligibility:
-have a diagnosis of cancer confirmed by an oncology health care provider
-currently in active cancer treatment
reside in the US or Puerto Rico
-meet federal poverty level eligibility guidelines

Intake Procedure:
Complete a brief interview by calling a CancerCare social worker. If determined eligible to apply, the social worker will email an individualized, bar coded application to the applicant.
