Community Nursing Services - Salt Lake Office

Delivers a variety of in-home health care services including home nursing, hospice, palliative care, speech as well as physical therapy, and homemaker assistance. Operates out of several locations throughout the state of Utah.

Speech Therapy
Description:

Licensed therapists provide speech therapy to individuals in the home, helping with swallowing and communication with family members and other caregivers.

Eligibility:
Homebound with a physician's referral. Does not serve children at this time.
Service Hours:
24/7
Intake Procedure:
The client's physician sends a referral to the agency. The agency will notify the physician whether it has the capacity to accept the client. Services typically begin within 48 hours of acceptance.
Fees:
Accepts Medicare, Medicaid, additional insurance plans and private pay. CNS Charitable Care funds may be available to assist as needed.

Palliative Care
Description:

Provides in-home medical care aimed at reducing suffering.

Homebound with a physician's referral.

Intravenous Medication
Description:

Provides infusion and intravenous (IV) therapies including medication administration, antibiotic and antifungal treatment, hydration and electrolyte therapy, immunoglobulin therapy (IVIG), autoimmune therapies, and total parenteral nutrition (TPN).

Description:

Provides in-home hospice care including symptom and pain management, home visits, counseling, spiritual support, medications, medical equipment, companion support, and bereavement services for patients and their families.

Immunizations
Description:

Provides child and adult immunizations. Provides mobile clinics for businesses from September to November, on-site back-to-school immunization clinics and school clinics. Check to see if they can schedule individual appointments for clients and families.

Physical Therapy
Description:

Licensed therapists provide physical therapy to individuals in the home, helping with mobility issues and fitting adaptive devices. Also provides occupational therapy services.

Homebound with a physician's referral. Does not serve children at this time.
