Health Well Foundation

Provides medical care expense assistance for covered diseases, medications, and medical treatments to adults and children for out-of-pocket medical expenses that health insurance does not cover. Also, provides incidental medical expense assistance for out-of-pocket costs associated with obtaining necessary medical treatment.

Description:

Provides financial assistance to patients for out-of-pocket costs associated with obtaining medical care including, transportation, lodging, etc.

Eligibility:
The patient must have a covered disease (a list of covered diseases is available on the website)
Have some form of health insurance that covers part of the cost of treatment (private health insurance, Medicaid, Medicare, or Tricare)
Medications must be listed under the disease fund associated with the patient grant (a medication lookup tool is available on the website)
Financial eligibility is determined by household income, household size, and the cost of living where the patient resides. Funds assist patients with a household income up to 300-500% of the federal poverty level.
The patient must be receiving treatment in the United States
