Intermountain Homecare and Hospice - St George

Provides home care and hospice services to those who elderly or homebound. Also provides a support group for individuals who have recently experienced the death of a loved one.

Hospice Care
Description:

Provides home care and hospice services to those who are elderly or homebound or facing a terminal illness.

Eligibility:
For those who are homebound and in need of home health care.
Service Hours:
M - F 8:30 am - 5 pm
Intake Procedure:
Must be referred by a doctor or caseworker.
Fees:
Accepts Medicaid, Medicare, private insurance,. and HMOs cover hospice services. Patients without adequate insurance or limited resources can apply for special financial assistance supported by donations to Intermountain Homecare.

Home Nursing
Description:

Provides skilled nursing care, physical therapy, and other support right at home. Available for post-surgery recovery, managing chronic conditions, or when needing ongoing medical care for those who are homebound.

Eligibility:
For those who are homebound and in need of home health care, with a doctor’s referral. May also qualify if patient has a medical condition that requires skilled nursing care or therapy at home.
Service Hours:
M - F 8:30 am - 5 pm
Intake Procedure:
Must be referred by a doctor.
Fees:
Accepts Medicaid, Medicare, and private insurance.
