Moab Free Health Clinic

Providing non emergency medical services to improve the quality of life for uninsured or under insured families through a community clinic, counseling services, health related volunteer opportunities, HIV testing, mental health support groups, navigator programs, foot care, a prescription assistance program, and sexually transmitted disease screenings.

HIV Testing
Description:

Offers confidential HIV/STD (HIV, Hepatitis C, Chlamydia, Gonnorhea) testing on a first-come, first-serve basis.

Eligibility:
Open to persons 14 years old and older, regardless of insurance status.
Service Hours:
Tu 1 - 2 pm
Intake Procedure:
Walk in; appointments are encouraged but not necessary.
Fees:
No cost. $15 donation appreciated per appointment, but not required.

Prescription Drug Patient Assistance Programs
Description:

The clinic helps patients apply to Prescription Assistance Programs (PAPs). Through Prescription Assistance Programs, many pharmaceutical companies provide now cost or significantly discounted medications to patients who qualify.

Eligibility:
Patients must have no public or private prescription drug coverage, including through government assistance programs (Medicaid, Medicare Part D, VA, etc.) Must be less that 200% of the federal poverty guidelines.
Service Hours:
M - F 9 am - 5 pm
Intake Procedure:
Call to schedule an appointment.
Fees:
$15 donation requested but whatever a patient can afford.

Community Clinics
Description:

Open to uninsured or underinsured persons who need to see a medical provider for general care. Appointments usually fill up about one week before each clinic

Eligibility:
Patients must have no public or private insurance coverage, including through government assistance programs (Medicaid, Medicare Part D, VA, etc.) Must be less that 200% of the federal poverty guidelines.
Service Hours:
M - F 9 am - 5 pm
Intake Procedure:
Call or walk in for an appointment.
Fees:
Minimum donation encouraged but not required.

Mental Health Related Support Groups
Description:

Support groups for mental health related struggles.

Eligibility:
Patients must have no public or private prescription drug coverage, including through government assistance programs (Medicaid, Medicare Part D, VA, etc.) Must be less that 200% of the federal poverty guidelines.

M - F 9 am - 5 pm

Podiatry/Foot Care
Description:

Podiatry/Foot Care services through the clinic can be available on an as-needed basis.

Eligibility:
Patients must have no public or private prescription drug coverage, including through government assistance programs (Medicaid, Medicare Part D, VA, etc.) Must be less that 200% of the federal poverty guidelines.
Service Hours:
M - F 9 am - 5 pm

Description:

General Counseling Services
Description:

Counseling services include but not limited to alcohol and drug abuse, general clinical therapy, anxiety, depression, domestic violence, and communication skills.

Eligibility:
Patients must have no public or private prescription drug coverage, including through government assistance programs (Medicaid, Medicare Part D, VA, etc.) Must be less that 200% of the federal poverty guidelines.
