Mountainlands Family Health Center - Provo Family Health Center

Provides the following services: Immunizations and well-baby checks, obstetrics/gynecology, community clinics, dental exams, cleanings, fillings and crowns, and mental health counseling. A reduced cost pharmacy is available for Mountainlands patients only.

General Dentistry
Description:

Provides low-cost dental care including exams, cleaning, x-rays, fillings, crowns, bridges, root canals, and prosthesis.

Eligibility:
For low-income individuals and families.

Accepts Medicaid, Medicare and private insurance. Offers a sliding scale fee for those without insurance.

Navigator Programs
Description:

Take Care Utah navigators/certified application counselors are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP.

Description:

Description:

Provides obstetrics for low-risk pregnancies. All deliveries are at Utah Valley Hospital. Women's health exams are available, including pap smears and breast exams.
