Provides support and education to those with disabilities to live independently through advocacy, specialized information and referral, education, nursing home transition services, mobility training, peer-to-peer help, social clubs, personalized services, and assistive technology loan.

Helps residents in nursing homes identify programs that can assist them in transitioning back to their home from a nursing home.
