San Juan Public Health Department

Provides services that promote the achievement and maintenance of optimal health for all people living in San Juan County. Services include restaurant/food sanitation, marriage licenses, food handling permits, death certificates, environmental hazards information, birth certificates, prenatal/postnatal home visitation, programs, early intervention for children with disabilities/delays, and WIC.

Environmental Hazards Information
Description:

Aids in the elimination of all environmental factors that lead to disease, lessen quality of life, and degrade the physical environment.

Death Certificates
Description:

Death certificates are available from 1905 to the present.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Intake Procedure:
Requests can be made in person and through the mail.

First certified copy $30. Each additional certified copy (ordered at the same time) $10.

Restaurant/Food Sanitation
Description:

Regulates food service establishments including restaurants, delis, food carts, cafeterias, caterers, concession stands, various other businesses, and organizations that engage in food preparation and service.

Birth Certificates
Description:

Birth certificates for births that occurred anywhere in Utah from 1905 to the present.

Eligibility:
Vital records are protected private information and are only available to the person listed on the record and the person's parents, current/surviving spouse, siblings, children, grandparents, and grandchildren.

Early Intervention for Children With Disabilities/Delays
Description:

Provides support for children with special health care needs. Behavior problems, autism, developmental delays, ADHD, and other special needs.

Eligibility:
For those with increased risk for chronic physical, developmental, behavioral, or emotional conditions and require additional services.

M - F 8 am - 12 noon & 1 - 5 pm

Call for case manager to schedule an appointment.

WIC
Description:

Provides information and education on the WIC program, which provides low-income women, infants and children with nutrition education, counseling, food and access to health care.

Eligibility:
Pregnant or breastfeeding woman, woman who has just had a baby, or child less than five years of age who are less than five years of age who meet income requirements.

T 8:30 - 11:30 am & 1 - 3 pm
Intake Procedure:
Walk in, Blanding, Montezuma Creek, and Monument Valley.

Marriage Licenses
Description:

Abstracts of marriage licenses and divorce records for anywhere in Utah from 1978 to 2010. To obtain a full, certified copy of the Utah marriage license, contact the county clerk's office in the county where the initial application was submitted. Certified divorce decrees are available at the Utah District Court where the divorce was granted.
