Promotes the safety, health, and independence of Tooele County residents 60 and older by supporting a coordinated system of services. Services offered include home delivered meals, non-emergency medical transportation, caregiver/care receiver support groups, senior ride programs, state Medicaid waiver programs, and Medicare information/counseling.

Home Delivered Meals
Description:

Provides nutritious, delivered meals to homebound seniors.

Eligibility:
Homebound Tooele County residents 60 and over.
Service Hours:
M - Th 8 am - 6 pm; F 8 am - 12 noon
Intake Procedure:
Call to schedule an assessment for determining program eligibility.
Fees:
Suggested $4 donation

State Medicaid Waiver Programs
Description:

Provides services to help older adults remain in their homes or other community-based settings. Help provided includes homemaker assistance, personal care, etc.

Eligibility:
65+ and eligible for both Medicaid and Medicare
