Give Kids a Smile is an annual dental event that provides low-income children without dental insurance dental care. Contact Community Health Connect for more information.
Eligibility:
Children 5-12 years old without dental insurance. Children older than 12 will be placed on a wait list. Families need to be at or below 150% of the federal poverty level.
Service Hours:
Office: M, W 9 am - 6 pm; Tu, Th 9 am - 4 pm Event: 3rd Sa in February
Intake Procedure:
Apply online. Fill out and return health history consent form before the last week of January.
