$25 for initial certificate, $10 for each additional copy; fee waivers available for those experiencing homelessness from local homeless service providers.

Program provides low-income women, infants, and children under 5 with nutrition education, vouchers for nutritious foods, breastfeeding support, a review of immunization records, and referrals to other helpful agencies.

Eligibility:
Pregnant or breastfeeding women, women who have just had a baby, or child less than five years of age who meet income requirements.

M, Tu, Th 8 am – 12:00 noon & 1 – 5 pm; W 10 am – 2 pm & 3 – 7 pm
