Utah Health Policy Project - Ogden

Assists underserved populations throughout Utah apply for health insurance through navigator programs and related advocacy groups. Also provides case management services to disabled adults through the Linkage program.

Navigator Programs
Description:

Take Care Utah certified health access assisters are available to help clients understand, apply for, and enroll in health coverage through the Marketplace, Medicaid, and CHIP. Appointments can be completed virtually, over the phone, or in person at one of the office locations.

Case/Care Management
Description:

Provides case management services to individuals with disabilities, assisting them connect to resources and fill out applications as part of the Linkage Coordinator Project.

Eligibility:
Utah residents 18 or older with a traumatic brain injury, mobility limitation, or an intellectual or developmental disability.
Service Hours:
M - F 9 am - 5 pm
Intake Procedure:
Submit a referral through the website. Self-referrals are accepted. Call and walk-ins are also accepted. Appointments highly encouraged.

Health Related Advocacy Groups
Description:

Advocates for the underserved population of Utah in areas of health care and provides information.
